What a good progress-monitoring template actually needs, and an example you can copy.

Progress monitoring is how you know whether a goal is actually working — not just at the quarterly report deadline, but week to week. A good template makes it obvious, at a glance, whether a student is trending toward the target or stuck.

What every progress monitoring record needs

  • A baseline.Where the student started, so "80% accuracy" means something — going from 40% to 60% is real progress even if the target is 90%.
  • A clearly defined target.Written the same way you'll measure it, so there's no ambiguity about what "met" looks like.
  • The skill being measured, specifically enough that a substitute provider could pick it up and know exactly what to assess.
  • Score or result for that session — raw score, percentage, or frequency count, whatever fits the measurement type.
  • Prompt level and setting."80% independent in a 1:1 setting" and "80% with verbal prompting in a whole-group setting" are very different results that a bare percentage alone hides.
  • Date.Obvious, but critical — a string of data points without consistent dates can't show a trend.

Goal: Student will identify the main idea of a grade-level passage with 80% accuracy.

Three data points already show a trend moving from 60% toward the 80% target, with the prompt level dropping from verbal to independent — exactly the kind of pattern that's easy to miss buried in a quarterly summary, but obvious once it's tracked weekly.
